This agreement records consent and participation terms for Goalers League. Separate Ontario concussion-awareness confirmations or other league safety requirements may also apply where required.

Participant Information

Enter the player details as they appear on the team roster if possible. We will still try to match the player if the spelling is slightly off, and the signer instructions below will update automatically from the participant’s date of birth as of the date this form is signed.

Parent/Guardian Information

For participants under 18, this section must be completed by the participant’s parent or legal guardian. For any minor, the parent/guardian is the legal signer.

Emergency Contact

This should be someone Goalers League can contact quickly during participation if reasonably necessary.

Medical Information

Provide any information Goalers League should know for participation, supervision, emergency response, and safety.

All acknowledgments below must be accepted before submission.
